The following forms and documents are needed by students who will be moving to one of the on campus housing facilities.
If you need to cancel housing for Summer or Fall/Spring, termination forms are available via the MyHousing portal. Log in to your MyHousing portal to access these forms. Pay close attention to the cancellation fee schedule, which is located on the last page of the housing contract.
University Housing Technology or Equipment Issues

The form is for University Housing related technology or equipment issues only. This includes front desk, checkpoints, mailrooms, learning centers and staff workstations within University Housing. For technology issues within the residence halls please contact MyResNet support at 855-813-7007 or email at [email protected] .
Appeals of housing and dining charges must be submitted online within 30 days of contract termination or move-out. Appeals will only be considered in cases where the student can provide acceptable documentation clearly establishing a substantial and unforeseeable change, as determined by the university. Appeals related to Room Reservation Fee or Termination Fees must be accompanied with a Termination Form. Approval/denial notifications will be sent out via your Georgia State email account within 10 days of receipt
Please allow seven to 10 business days for your parking appeal to be processed.
University Housing Cleaning Agreement
Please take the time to talk with your roommates regarding how the apartment/suite will be cleaned in preparation for check-out. In the space provided, please indicate which individuals will accept responsibility for the cleaning of each area. Please note that failure to properly clean each living space may result in a minimum cleaning charge of $50.00 per resident. Please make sure all roommates are in agreement with the cleaning arrangement before submitting this form.
Bedroom: Each resident is responsible for cleaning their own bedroom.

Mandatory Meal Plans
Students living in Patton, Piedmont Central or Piedmont North residence halls are automatically enrolled in a 7-day mandatory meal plan. Mandatory meal plans cannot be cancelled as long as you are living in the above mentioned residence halls. Cancellation fees are structured as listed below.
Voluntary/Non-Residential Meal Plans
Voluntary/Non-Residential meal plans can be cancelled anytime before Sept. 17. Cancellation fees are structured as listed can be found on the Panther Dining page.
Emergency School Closure Refunds
If the Board of Regents deems it necessary to close campus, we will follow their guidelines.
